Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling into the elevator shaft. a) Freight Elevator, on or about 11/03/99: The Freight elevator was not equipped with interlocking devices to prevent the hoistway doors from opening when the elevator platform was not parked at that level and to prevent the elevator platform moving when any of the hositway door are open. Abatement documentation must be submitted for this item.
