Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck-by the load should it slip off the hook: a)In the boiler room, on or about 12/15/99: Overhead hoist system was used without safety clip on lifting hook. ABATEMENT NOTES: Among other methods, one feasible and acceptable abatement method to correct this hazard is to install or repair safety clips on the hook, and follow the requirements of the American National Standard for electric chain hoists (ANSI/ASME B30.16-1987). Abatement certification must be submitted for this item.
