Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck-by the falling load should the attaching points, hoist, or lifting hook fail: In the fabrication shop, on or about 01/26/00: Underhung hoists and cranes have not been inspected and tested prior to initial use, and are not being inspected on frequent and periodic basis. a)North end of fabrication shop, Robin Meyers 2 ton underhung crane. b)South end of fabrication shop, Twin Cities 4 ton underhung crane. c)South end of fabrication shop, Ingersol Rand 1/2 ton hoists (2 ea). ABATEMENT NOTES: Among other methods, one feasible and acceptable method to abate this hazard is to: Conduct initial, frequent, and periodic inspections of the hoists and support structures for wear, deterioration or malfunction as per ASME/ANSI B30.16- 1987. Abatement certification must be submitted for this item.
