Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of amputation from high pressure water
operating at 8000 PSI
a. LCP chemical site, decon area on or about 08/31/00: An employee was
operating a high pressure water jet at a working pressure of 8000 PSI.
Components were not inspected on a daily basis and worn or deteriorated
parts were not replaced. The high pressure hose had visible signs of
excessive
wear and the inner cords was visible on several areas of the hose.
Among others, one feasible and accepted abatement method is to follow the
recommended
practices for the use of high pressure waterjetting equipment published by
the WaterJet
Technology Association (WJTA), including but not limited to, the following:
1.(Sec. 7.2) Provide operator training including all the sections in the
WJTA
recommended
practices.
2.(Sec. 11.1) Require daily inspection of the equipment and replacement of
any worn part
or equipment.
