structures, below the hook lifting devices that were not rated, and
frequent
and periodic inspections were not being completed.
1. Press room, 4000 lb Coffing hoist used on a structure rated at 1000 lbs,
two below the hook lifting devices without rated load capacity markings and
no inspections were performed.
2. Heat treat area, 2000 lb Coffing hoist used on a structure rated at 1000
lbs.
3. Barrel room, 2000 lb Coffing hoist used on a structure rated at 1000
lbs.
ABATEMENT NOTES:
Among other methods, one feasible and acceptable methods to abate these
hazards is to:
Conduct initial, frequent and periodic inspections of the hoists and
support
structures for wear, deterioration, or malfunction in accordance with ASME
B30.16-1987 and ASME B30.20-1985.
