Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARDS OF AN UNSTABLE LADDER: a)Grinding Area, On or about 2/7/01: A Werner Fiberglass extension ladder was used without any slip resistant "feet", and was set up with its siderails on an uneven surface. Among other methods, several feasible and acceptable abatement methods to correct these hazards are to: 1.Install slip resistant "feet". 2.Utilize a foot ladder board. 3.Place the ladders siderails on an even surface. 4.Follow the requirements set forth in the American National Standard (ANSI) A14.5.
