Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to A FALL HAZARD: a) Komatsu Lift Truck Type LT, serial # 506763A, on or about 3/26/01: Platform used by employees was not secured to the forks of the truck. ABATEMENT NOTE: Ensure that the platform is secured to lift truck per American National Institute, ANSI Z56.1- 1993, Section 4.17.3(b). b)Komatsu Lift Truck Type LT, serial # 506763A, on or about 3/26/01: Platform used by employees was not equipped with a midrail. ABATEMENT NOTE: Ensure that fall protection in the form of body belts, lanyards and/or fully functioning guardrails are used per American National Institute, ANSI Z56.1- 1993, Section 4.17.2(c). Abatement certification must be submitted for this item.
