recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a STRUCK-BY HAZARD:
a)Warehouse "C", Testing Dept., on or about 05/10/01: Employees were
exposed to a struck-by hazard when using the high pressure test box to test
hydraulic hose assemblies. The high pressure test box had two of three
total
hinges for the cover broken and did not have a means to keep the cover
closed if a hose burst.
ABATEMENT NOTE: Ensure that the cover for the high pressure test box
is maintained in an operable and safe condition per Drawing Number C-
46674, Title-Details Hose Pressure Tester, The Gates Rubber Company.
