Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being pulled into and crushed by the rotating parts of a pipe threading machine: a)Stewart Elementary School, east side of new wing - An employee was at risk of falling approximately 30 feet while standing on a wood pallet being lifted by the forks of a Durante Traverse Lift TL 8042, rough terrain forklift truck, s/n 8C1153- CUC. Fall protection was not provided; on or about 10/05/00. ABATEMENT NOTES: Among other methods, one feasible method to abate this hazard is: 1. Utilize only platforms designed for use of elevating personnel with forklifts. These platforms must meet the requirements set forth in ANSI/ASME B56.6- 1987 such as, but not limited to, fall protection for employees. 2. Train all employees about the hazards of falls associated with improper elevating methods, and the proper application and use of safety platforms. ABATEMENT VERIFICATION IS REQUIRED AS OUTLINED IN PART 29 CFR 1903.19D
