Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing/amputation hazards while operating press brakes: a) Shop area - An employee was exposed to a crushing/amputation hazard while operating a Heim press brake. The employer failed to provide a "safe distance" program for operators in lieu of traditional guarding. On or about 07-20-00. b) Shop area - An employee was exposed to a crushing/amputation hazard while operating a Di-Arco press brake. The employer failed to provide a "safe distance" program for operators in lieu of traditional guarding. The employee sustained injury to his left index finger as it was caught in the die during operation. Occurred on or about 04-06-00. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to require and enforce the use of "safe work practices" and alternative safeguarding techniques for the operation of power press brakes during custom jobs as described in OSHA Directive CPL 2-1.25 (Guidelines for Point of Operation Guarding of Power Press Brakes). NOTE:ABATEMENT DOCUMENTATION REQUIRED FOR THIS ITEM. See page 2,"Notification of Corrective Action" for instructions.
