Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling from a unstable elevated work platform: a) On or about March 7, 2000, an employee marking "rose meat" while standing on an unstable 3' elevated work platform due to two missing and one improperly adjusted floor levelers. Abatement Note: Among others, one feasible and acceptable abatement method to correct this hazard is to conduct regular inspections of all elevated platforms to insure there stability. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of amputations and or crushing injuries while placing hands and/or arms in the danger zone of a moving conveyor system to activate it's "stop" button. a) Employees standing on an elevated work platform in the "trolley return area" are required to place their hand/arm in close proximity to the moving conveyor system to activate it's "stop" button. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to reconfigure the "stop" button so the operator does not have an obstruction (conveyor assembly) to contend with when activating it's "stop" button as identified in ASME B20.1. paragraph 5.12(b). Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification
