were causing or likely to cause death or serious physical harm to

were exposed to acid from the multi-media filter tank when the tank failed:
a)APU Waste Treatment Room:  The ECO-TEC Multi-Media Filter Tank was
replaced with a plastic tank which was not designed and fabricated to
manufacturer specifications, and an employee was sprayed with nitric and
hydrofluoric acid when the tank ruptured, on or about 10/06/99.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to follow the ECO-TEC operating manual and replace the tank with a
multi-media tank designed
by ECO-TEC to APU system specifications.
