recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of inhaling glutaraldehyde and splashing
hazard due to spills of
solution leaking and spilling onto the floor under the endoscopic washers.
a)Endoscopy Equipment Room:  Endoscopic washers (Custom Ultrasonics 83 and
Custom Ultrasonics 83 Plus) and the Vapor Recovery Unit (Custom Ultrasonics
93) were not maintained within manufacturer's specifications, which has
led to
leaks whereby employees have been splashed with a solution containing about
2.6glutaraldehyde, and may have been overexposed while cleaning up spills
of the glutaraldehyde solution, on or about October 15, 1999.
One feasible and acceptable abatement method to correct this hazard is to
follow the Custom
Ultrasonic's Routine Maintenance Procedures.
