by excavation operations, support systems such as shoring, bracing or
underpinning was not
provided to ensure the stability of such structures for the protection of

a)On 05/06/2000 employees performing a disinterment were not protected
from adjoining
excavated walls, by a protective system such as but limited to: shoring,
sheeting or bracing.
Employee was endangered by the excavated walls and trapped in a cave-in.

METHOD TO CORRECT THIS HAZARD IS TO FOLLOW THE GUIDELINE SET

of the excavation (grave).
Protection to prevent soil from falling/rolling from the excavated face

On 05/06/2000 an inspection of an excavation (grave) that was to become a
disinterment
operation was not conducted by a competent person to ensure that the
excavation did or did not
need a protective system prior to employees entering the excavation. An
employee was exposed
to the hazard of cave-in while the employee was performing the
disinterment, trapping him in
the excavation.
