deficiencies:
(1)  The stop button on the pendant control was not operational.
(2)  The latch on the hook was not operational.
(3)  Preventative maintenance inspections were not completed in accordance
with
the manufacture instructions on a monthly or periodic basis.
(b) Fabrication/Weld Shop:  The 5 ton Load Lifter Hoist had the following
deficiencies:
(1)  The stop button on the pendant control was not operational.
(2)  Preventative maintenance inspections were not completed in accordance
with
the manufactures instructions on a monthly or periodic basis.
