Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to multiple injuries from falling hazard when employees are elevated on platforms when used with a powered industrial truck and the platform is not secured to the lifting carriage and/or forks and not provided with a standard guardrail system: (a) Allison Park Industrial Complex, Building 6 A: A standard wood pallet, placed on the Yale forklift, was used to elevate personnel approximately 21 feet to the roof. The pallet was not designed to lift personnel, was not equipped with a standard guardrail, and was not secured to the forks. Among other methods, one reliable and acceptable abatement method to correct this hazard is to use an alternative means of placing material and personnel on the roof (i.e. ladders and hoists).
