Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of falling and/or uncontrollable loads: a) Mill Area; In Front of Furnace to Beginning of Three High Area; Cleveland 5 Ton Hoist; Employees removing stuck billets from the rolling mill side pull the billets with the hoist. In addition, an inspection program and preventive maintenance program had not been developed and implemented in accordance with the manufacturer's and ANSI specifications. b) Cooling Bed Bay Area; Three Rivers Crane Inc. 2 Ton Underhung Crane; The following deficiencies existed: (1) The entire right hoist mechanism was replaced, including the motor. An additional pendant control (the electric cable to the pendant had several splices in it) was added to operate the right hoist, thus bypassing the original pendant controls. The operator has to use both pendant controls when lifting the rebar load. When the right hoist mechanism was replaced, the lower hoist block was replaced with a two sheave block (the original block had four sheaves - the left lower block still has the original four sheave block). As a result of these modifications, the load comes up uneven and the cable on the left hoist wears on the lower left hoist block (the block has grooves worn in it). The modifications were made without the manufacturers approval. (2) An inspection program and preventive maintenance program had not been developed and implemented in accordance with the manufacturers's and ANSI specifications. (3) The homemade spreader bar attached to the lower hoist blocks was not proof tested. In addition, the homemade spreader bar did not have a capacity marking, nor it's own weight marked on the bar. Among other methods, one feasible and acceptable abatement method to correct this hazard is to establish procedures in accordance with the manufacturer's and American National Standards Institute specifications.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to railcars striking billet arm machine: (a) Railroad Track Area; A "close clearance" sign was not posted in the area of the railroad tracks where the billet pusher arm machine extends out of the building next to the tracks. The end of the billet pusher arm machine was approximately thirteen inches away from a railcar on the tracks. In addition, a blue flag warning system was not implemented to prevent the railroad cars from being moved during loading/unloading operations. Among other methods, one feasible and acceptable abatement method to correct this hazard is to post a "close clearance" sign and install a blue flag warning system.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to railcars striking a six inch gas line: a) Billet Bay Area at end of Railroad Tracks; A rail stop was not provided at the end of the track to prevent the railcars from striking the back wall and a six inch main gas line. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install a bumping post at the end of the tracks.
