Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by a defective vehicle and/or loose grindings: (a) Plant: The electrical interlock for the door on the diesel powered Mustang, (Bobcat), Serial #SF97E01368, Model #2040, was defeated thereby permitting employees to operate this piece of equipment without the safety door. Employees were exposed to being struck by falling grindings. In addition, employees were not trained in the safe operating methods for this piece of equipment nor had employees reviewed the "Operating Manual" as recommended by the equipment manufacturer. Among other methods, one feasible and acceptable abatement method to correct this hazard is to properly install the electrical interlock and to train all employees in the safe operating methods as outlined by the manufacturer.
