load to fall:

ladder truck
equipped with a crane hook attachment with wire rope that was broken,
crushed and
damaged.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to inspect the wire rope prior to use of the device and develop
procedures
and/or provide training to employees on the proper techniques to reeve
wire
rope and
inspect wire rope.
