North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)facility; winding - monorail system with a Yale 2 ton hoist has not been load tested. b)facility; assembly - monorail system with a CM Model W 2 ton hoist has not been load tested. c)facility; disassembly - monorail system with a Dayton 1/2 ton hoist has not been load tested. d)facility; disassembly - monorail system with a Budgit 1/2 ton hoist has not been load tested. e)facility; disassembly - monorail system with a Yale 1 ton hoist has not been load tested. REFERENCE: ANSI B30.11-1980; paragraph 11-2.2.2(a) Rated load test - Prior to initial use, all the equipment mentioned in the above paragraph is to be tested... NOTE: Among other methods, one feasible and acceptable method to correct this hazard is to load test all the cranes.

REFERENCE: ANSI B30.11-1980; paragraph 11-2.1.1(a) Initial inspection - Prior to initial use, all new, altered cranes shall be inspected by a designated person.. NOTE: Among other methods, one feasible and acceptable method to correct this hazard is to have all the monorail systems initially inspected.

Reference: ANSI B30.16-1981; paragraph 16-2.3.3(b) Adjustments and repairs - Adjustments shall be made to assure correct functioning of components..such as (3) limit devices of electric hoists. NOTE: Among other methods, one feasible and acceptable method to correct this hazard is to have the limit switch repaired.e
