that were causing or likely to cause death or serious physical harm to

a) The hazard of falling approximately 14 feet from the elevated platform
while not
maintaining a firm footing on the platform floor and using the mid rail to
achieve additional
height to install the insulated panels to the roof of the Faith Lutheran
Church in Great Falls,
Montana.
Among other methods, one feasible means and acceptable abatement methods
to correct this
hazard would be to assure that employees maintain a firm footing on the
platform and not
use planks, ladders, etc., to gain additional height unless secured by
body belt and lanyard as
stated in ASME/ANSI 56.6 - 1987, Section 5.15 - Elevating Personnel.
Abatement certification and documentation are required for this item.
