Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to employees in that the an employee was exposed to: (a)The hazard of possible unexpected movement of the machine and/or the load due to the operator being more than 25 feet from the controls of the forklift while the forks were elevated with personnel on the platform. Among other methods,one feasible and acceptable method to correct the hazard would be to follow the guidelines set forth in ANSI B.56.6 1992, Section 6.2 and to keep the operator within 25 feet and visual contact with the controls. __ Abatement certification or documentation is NOT required for this item. __ Abatement certification is required for this item. x_ Abatement certification and documentation are required for this item. __ Warning tag (or citation copy) required on this equipment before moving.
