Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) The hazard of falling approximately 13 feet while working off a unsecured, unguarded wooden platform that had been raised in place by the forks of a Taylor forklift located at the corner of Shiloh Rd. & Grand Ave. in Billings, Montana. Among other methods, one feasible and acceptable abatement method to correct this hazard is to design and build a safety platform and secure it to the forklift as stated in ASME/ANSI B56.1-1993. __ Abatement certification or documentation is NOT required for this item. X_ Abatement certification is required for this item. __ Abatement certification and documentation are required for this item. __ Warning tag (or citation copy) required on this equipment before moving.
