Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) The hazard of falling due to equipment tip over while working on a metal platform that was more than 10 inches wider than the "Gehl" rough terrain forklift and the operator was not within 25 feet and visual range. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to design a safety platform which meets the requirements of ASME B56.6-1992 and ASME B56.6a-1994 Addenda and provide an operator within 25 feet of the machine when lifting personnel. __ Abatement certification or documentation is NOT required for this item. __ Abatement certification is required for this item. x_ Abatement certification and documentation are required for this item. __ Warning tag (or citation copy) required on this equipment before moving.
