exposed to the hazards of being struck by rigging gear.

Employees were exposed to serious injury when they (1) unexpectedly
encountered
difficulty in obtaining release of a concrete form from cured concrete;
(2) unilaterally
varied from the employer's rigging instruction; (3) unilaterally bolted
the rigging gear
to an unapproved rather than the approved anchor point; and/or (4) failed
to use washers
to prevent the bolts from being pulled through the form when force was
applied to the
rigging.
