Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The hazard of being caught between descending or ascending elevator car and shaft wall due to the fact that the hoistway doors would open while the elevator was moving. Construction site: 622 3rd Avenue, New York. On or about 08/24/00 a)Freight / personnel elevator # 30970, used by construction contractors to access upper floors had defective hostway doors. Among other methods, one feasible and acceptable method to correct this hazard is to repair the shaftway door electromechanical interlocks. As referenced in Reference: ASME A.17.3 - 1993 2.7.1
