Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to: Being crushed or asphyxiated by excavation collapse while
working in
excavations greater than 5 feet deep that were not shored or sloped to the
angle of repose.
a)On site.
Employees entered a grave that was approximately 8 feet deep in order to
cut
ridges to allow the casket straps to slide out, and the excavation had a
vertical
wall which did not have a protective system of any kind on that wall,
thereby
exposing those employees to trench collapse, on or about 10/3/00.
ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement
method
to correct this hazard is to follow the guidelines set forth in 29 CFR
1926.651-652.
