General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following condition(s): a) Caustic Tank Area - Scrubber Seal Tanks for the #1 and #2 Lime Kilns were altered from their original API-650 design specifications. The original design of 16 bolts securing the manhole cover was changed to a wing-nut mechanism which required periodic re-seating. This exposed employees to the hazard of spraying, leaking and/or spilling caustic. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: Assure that the design of the manhole cover conforms to the tank's original design specifications as stated on Drawing #IF-583-T and API 650. Should the employer still wish to make modifications to the manhole cover securing devices, proper engineering analysis and documentation must be performed.ion
