North Carolina General Statute 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish to
each of his employees conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:
a) facility  --  employees were exposed to ingoing nip
points on the 3-roll impregnator in the pilot plant due
to the employer did not instruct each employee in the
recognition and avoidance of unsafe conditions and unsafe
acts applicable to his or her work environment to
control or eliminate any hazards or injury.  An employee
was injured on June 15, 1999, when her hand was caught
between two metal rollers while she was cleaning the
rollers with the machine running.
"Among other methods, one feasible and acceptable
abatement method to correct this hazard is to thoroughly
train each employee in the safe operation of the
machinery, have the employee read the standard operating
procedure on the machinery, and sign a document that they
have been trained."
