40.1-51.1(a) of the Code of Virginia:  Webster & Webster, Inc., did
not furnish a place of employment which were free from recognized
hazards that were causing or likely to cause death or serious
physical harm to employees in that employees were operating a 2 Ton
Electric Powered Chain Hoist, which is located in the shop, which
exposed them to:
(a)  Shocks by having an electrical cord which was frayed in
several areas, one area having been taped and, by having a push
button station taped together because of cracked and missing pieces
of the cover.  Among other methods, one feasible and acceptable
abatement method to correct this hazard is to install a new
electrical cord and a new push button station such as that called
for by ANSI B30.16-1987, American National Standard Safety Code for
Hoists.
(b)  Contusions and/or fractures by not having a hook equipped with
a latch for lifting heavy equipment.  Among other methods, one
feasible and acceptable method to correct this hazard is to install
a new latch which is equipped with a latch hook such as that called
for by ANSI B30.16-1987, American National Standard Safety Code for
Hoists.
(c)  Contusions and/or fractures by not conducting frequent
(monthly) visual inspections of all controls and operating
mechanisms, safety devices, hooks, chains and chain stretch.  Among
other methods, one feasible and acceptable method to correct this
hazard is to conduct monthly inspections such as that called for by
ANSI B30.16-1987, American National Standard Safety Code for
Hoists.
(d)  Contusions and/or fractures by not conducting periodic
(annual) documented inspections of loose bolts, excessive wear of
chain, chain stretch, hook, electrical apparatus to include push
button station, supporting structure, and end connections of load
chains.  Among other methods, one feasible and acceptable method to
correct this hazard is to conduct annual inspections such as that
called for by ANSI B30.16-1987, American National Standard Safety
Code for Hoists.
(e)  Contusions and/or fractures by not providing a written, dated,
and signed annual inspection report and record to indicate required
inspections have been completed for items inspected during an
annual inspection.  Among other methods, one feasible and
acceptable method to correct this hazard is to provide this
document after performing annual inspections such as that called
for by ANSI B30.16-1987, American National Standard Safety Code for
Hoists.
