Title 40.1-51.1(A), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that: (a) Located at 6363 Center Drive, Norfolk, Virginia, the employer did not insure that the operating manual was stored on the aerial lift/hoist truck, #29 5X29, and that the operating manual was available for employee use in setting up and operating the aerial lift, exposing the employees to various hazards to include hit by, struck by, and tip over associated with the improper use and setup of the aerial lift/hoist truck. One feasible and acceptable method among others to correct this hazard is to adhere to the American National Standard Institute, ANSI/SIA A92.2-1990, section 8, paragraph 11, which state that the owner and user shall insure that the operating manual(s) is stored on the mobile unit.

Title 40.1-51.1,(A), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that : (a) Located at 6363 Center Drive, Norfolk, Virginia, the employer had not insured that the front right and left outriggers/stabilizers had been fully extended out before and during the use of the aerial lift/hoist truck, #29 5X29, exposing employee to fall hazards resulting from possible tip overs from the aerial lift/ hoist truck becoming unstable during operations. One feasible and acceptable method among others to correct this hazard is to adhere to the American National Standards Institute ANSI?SIA A92.2-1990, section 10, paragraph 8(3), which state to insure that outriggers and stabilizers are use if the manufacturers instructions require their use.

Title 40.1-51.1(A), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that:(a) Located at 6363 Center Circle Drive, Norfolk, Virginia, the U-bolt clips on the hoisting rope for the aerial lift/hoist truck, #29 5X29, did not have the U-bolt section on the dead or short end and did not have the saddle on the live or long end of the rope. Additionally, the minimal of 3 U-bolt clips were not used on the 3/8" wire hoist rope, exposing the employees to hit by/struck by hazards from possible load lifting failure on the aerial lift/hoist truck. One feasible and acceptable method among others to correct this hazard is to adhere to the American National Standard Institute, ANSI B30.5, section 5.1.7.3(b) which states U-bolt clips shall have the U-bolt section on the dead or short end, and the saddle on the live or long end of the rope. spacing and number of all types of clips shall be in accordance with the clip manufacturers recommendation.
