Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer failed to
provide a place of employment which was free from recognized hazards that
were likely to
cause death or serious physical harm to employees:
a. On or about December 1, 1999, at the jobsite located at 4740 West 95th
Street in Oak
Lawn, Illinois, an employee working from a platform supported by a rough
terrain forklift
truck was exposed to injuries from falls due to improper elevating of
personnel. The proper
method is required by the American National Standards Institute's standard
for rough terrain
forklift trucks. (ANSI B56.6-1992).
NOTE:  29 CFR 1903.19 Requires that you provide certification and
documentation of
abatement for this item.
