Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer failed to provide a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees: a. On or about January 18, 2000, at the jobsite located at 1344 South Morgan Street in Chicago, Illinois, an employee working from an aerial lift was exposed to injuries from falls due to the lack of (personal) fall protection systems required by the American National Standards Institute's standard for boom-supported elevated work platforms. (ANSI A92.5- 1992).
