Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer failed to
provide a place of employment which was free from recognized hazards that
were likely to
cause death or serious physical harm to employees:
a. On or about May 25, 2000, at the jobsite located at 1400 North
Milwaukee Avenue in
Chicago, Illinois, an employee working from a platform supported by a
rough terrain forklift
truck was exposed to injuries from falls due to improper elevating of
personnel. The proper
method is required by the American National Standards Institute's standard
for rough terrain
forklift trucks. (ANSI B56.6-1992).
A feasible and useful means of abatement includes, but is not limited to
the following action:
1. Ensure that all employees who work with the rough terrain forklift
truck follow the proper
methods required by parts 5.15 (Elevating Personnel) and 8.24 (Platforms
for elevating
personnel) in ANSI B56.6-1992.
