Section 5(a)(1) of the Occupational safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a. On or about September 7, 2000, at Hines V.A. Hospital, employees working on OR-4 and OR-6 were exposed to a fire hazards and/or smoke inhalation due to the openings (approximately 1-2 inches) in the fire rated doors which will void the protection required by the National Fire Code (NFPA #80, 1995) for Fire Doors and Fire Windows. A feasible method to correct the hazard is: Provide complete seal of the doors to maintain the integrity of a rated fire door. Reference: NFPA 80, 1995.
