and Health Act of North Carolina:  The employer did not furnish to
each of his employees conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:
a)site, plant 1 (main building) vulcanizer area, by not
developing and implementing standard operating procedures
for the safe operation of the vulcanizer (Southwest
Welding & Manufacturing Co., S/N 8641).  As a result,
proper workpractices in locking the door were not
followed by personell, i.e., guide bars and locking arm
were not utilized.

and Health Act of North Carolina:  The employer did not furnish to
each of his employees conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:
a)site, plant 1 (maint building) vulcanizer area, for the
lack of proper maintenance and repair of the large
vulcanizer door, referred to as the "big door", and its
seal.  Several signs indicative of poor maintenance
thereby leading to improper closing and locking of the
vulcanizer door include, but are not limited to: the
necessary use of a forklift to open and close the door;
worn-out gasket seal leading to occasional steam leaking
and running the locking collar past the guide bars; and
a potentially sagging door as evident from cutting and
welding repairs to the upper section of the hinge.

authorization for use of the National Board "R" symbol
stamp or an ASME Code symbol stamp.
