North Carolina General Statute 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish to
each of his employees conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:

(a)Welding shop, monorail system used by employees had not
been load tested prior to initial use.
(b)Plate fabrication, monorail system used by employees had
not been load tested prior to initial use.
(c)Milling area, monorail system used by employees had not
been load tested prior to initial use.
(d)Sawing area, Underhung crane used by employees had not
been load tested prior to initial use.
Note:  Among other methods, one feasible and acceptable
abatement method to correct this hazard is to have the
above systems load tested.
