free from recognized hazards that were causing or likely to cause death or
serious physical harm

On or about May 19,2000 employee was using a Rigid 300 electric pipe
threading machine
which was not provided with a constant pressure foot threader control
switch as required by
American National Standards Institute B208.1-1982, section 4.4.
Among other methods, one feasible and acceptable method of abatement is:
Provide the pipe threading machine with a constant foot pedal control
switch in accordance with
ANSI B208.1-1982, section 4.4.
