Section 5(A)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish to employees employment and a place of
employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees:
a)  On or about 12/19/00, an employee was exposed to the hazard of being
crushed or seriously
injured from concrete walls measuring approximately 106 inches x 154
inches x 6 inches and
weighing in excess of 10,200 lbs that were being unloaded from the sides
of an A-frame truck.
A concrete wall of similar size was being unloaded from the left side of
the truck with a crane
and both sides of the truck were unsecured.  The slings and chains used to
secure the walls onto
the truck were unhooked.  A concrete wall from the right side of the truck
fell from the truck
and hit the truck driver who was in a location between the crane and the
truck.
Feasible means of abatement include, but are not limited to:
1. Secure the side of the A-frame that is not being unloaded to prevent
the concrete walls/slabs
from falling off the truck while the crane in unloading the other side.
2.  Secure the area where the crane is being operated by using barrier
tape to prevent workers
from walking or standing in the immediate area where the load is being
lifted.
3. Designate a person to watch the loading and unloading of the walls to
prevent employees from
walking in and around the area.

REQUIRED FOR THIS VIOLATION.
