Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in the form
of being struck by falling materials in that employees were exposed to:
(a)Job Site at 4500 Progress Place, Springfield Missouri - On or about
January 5, 2001,
Nylon slings were being used to hoist a dust collection fan unit with
slings rigged
with a choker and attached around sharp edges on the fan unit without
protective pads
installed to prevent cut through. Among other methods, recognized and
feasible means
of abatement to correct this hazard include, but are not limited to:  Use
a shakle or
padding at the three connection end on the fan unit when lifting or moving
the dust
collection fan unit to prevent damage such as cutting or tearing, as
required by the
American Society of Mechanical Engineers (ASME) standard B30.9 (Slings), &
section 9-9.5, Operating Practices paragraph (g).
Note:  Abatement certification is required for this violation (using the
CERTIFICATION OF
CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating
that
abatement is complete must be included with your certification.  This
documentation may
include, but is not limited to, evidence of the purchase or repair of the
equipment,
photographic or video evidence of abatement, other written records.
