40.1-51.1(a) of the Code of Virginia:  Virginia Industrial Services, Inc.,
did not furnish a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in
that employees were operating a Chicago Press Brake, serial #P7876, a
Cincinnati Press Brake 3-50 and, a Cincinnati
Shear, which are located in the fabrication and machine shops, which
exposed them to:
Amputation by not providing a cover for the foot treadles on the press
brakes to prevent accidental actuation by falling
or moving objects, or by someone stepping on it.  Among other methods, one
feasible and acceptable abatement
method to correct this hazard would be to provide a cover over the foot
treadles for the press brakes such as that called
for by ANSI B11.3-1982, American National Standard Safety Code for Power
Press Brakes.
Contusions and abrasions by not providing adequate safeguarding to the
back of the shear and press brakes to prevent
the operator and other employees from injury by being too close to the
stock as it completes its cycle.  Among other
methods, one feasible and acceptable abatement method to correct this
hazard would be to provide an awareness barrier
on the back of the shear and the press brakes to prevent employees from
coming too close to the machines such as that
called for by ANSI B11.4-1983, American National Standard Safety Code for
Shears and, ANSI B11.3-1982,
American National Standard Safety Code for Power Press Brakes.
