General-duty citation text
40.1-51.1(a) of the Code of Virginia: City of Staunton Public Works Department, did not furnish a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are operating a 1 Ton Yale Electric Powered Hoist, which is located in the welding shop, which exposes them to contusions and/or fractures by: (a) failing to have the load rating posted on the supporting structure of the crane. The load rating shall be at least equal to that of the hoist. Among other methods, one feasible and acceptable abatement method to correct this hazard is to post the load rating on the structure such as that called for by ANSI B30.16-2.1.3.6-1987, American National Standard Safety Code for Hoists. (b) failing to conduct visual daily and documented monthly inspections of all controls and operating mechanisms, safety devices, hooks, chains and chain stretch. Among other methods, one feasible and acceptable method to correct this hazard is to conduct monthly inspections such as that called for by ANSI B30.16-1897, American National Standard Safety Code for Hoists. (c) failing to conducting documented annual inspections of loose bolts, excessive wear of chain, chain stretch, hook, electrical apparatus to include push button station, supporting structure, and end connections of load chains. Among other methods, on feasible and acceptable method to correct this hazard is to conduct annual inspections such as that called for by ANSI B30.16-1987, American National Standard Safety Code for Hoists.
