recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a crushing hazard:
a) Rear maintenance lot,  41 Village Park Road, Cedar Grove,NJ.
Employees were not provided with a safe means of supporting the
bucket/lifting arms of the
heavy equipment (Caterpillar 950B Wheel Loader, NJ license plate UA-31-5).
Employees were
exposed to the hazard of being crushed when the improperly supported
bucket fell.
Violation occurred on or about 02/05/2000
Among other methods, one feasible and acceptable method to correct the
hazard is to ensure that
the support equipment as set forth in the equipment owners manual is
provided and utilized by
employees and to provide specific training to employees.
ABATEMENT NOTE:  This violation requires documentation and certification
of abatement.
