employer did not furnish employment and a place of employment which were
free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees:

Condition:Employees operating and servicing the metal cutting "Startrite"
H225
horizontal band saw were exposed to amputation injuries as they were
not provided training to ensure safe operation and proper maintenance
of the saw.
Among others, one feasible and acceptable method to correct this hazard is
to train the
employees in the proper maintenance and operation of the saw using the
Manufacturers
Operation and Maintenance Manuals with the employers specific instruction
incorporated.
Abatement documentation (photographs, receipts, copies of programs,
training
certification, etc.) is required for this item.
