Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a falling hazard:
a) Warehouse:Employee raised on a pallet by a forklift truck.  The height
was
approximately four tiers high.  The employee did not use an appropriate
safety platform
or other fall protection, and was exposed to a fall to the concrete floor
below.
Violation observed on or about 6/1/01.
ABATEMENT NOTES:AMONG OTHERS, ONE SUITABLE METHOD FOR ABATING
THIS HAZARD CONDITION IS:
1)Utilize only platforms designed for use in elevating personnel with
forklifts.  These platforms
must meet the requirements set forth in ANSI/ASME B56.1-1993.
2)Train all employees about the hazard of falls associated with improper
methods, and the proper
application and use of safety platforms.
