Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees; specifically: Adequate fall protection was not provided for an employee working from an elevated work platform attached to a CAT 910 loader unit, in that the platform had no mid-rail and a safety harness and lifeline was not provided for the employee.
