Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that:
(a)At the worksite at 1942 North Commerce Street, Milwaukee, WI 53212 the
worker installing windows from the Dynalift had no personal fall arrest
system,
was using a fabricated wooden platform, and was kneeling on the top rail
of the
platform which was not approved for lifting of personnel by the truck
manufacturer.  Feasible abatement would be to obtain approval from the
lift truck
manufacturer before lifting personnel, and comply with the American
National
Standards Institute (ANSI) B56.6b - 1998 Standard for Rough Terrain
Forklift
Trucks which specifies the design, manufacture and use of all attachments
for
Rough Terrain Forklift Trucks.
Certification, with date and method of abatement required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment
