On or about September 26, 2001 through October 17, 2001 (according to
employee
interviews), employees were elevated and working inside a shop made wooden
personnel
lift used with a Gradall All Terrain Fork Lift.  Without wearing a
personal fall arrest
system (harnesses with attached lanyards).  This condition exposed the
employees to a
fall hazard of up to 30 feet to ground level.
One or more acceptable feasible abatement methods could be achieved by
following the:
Safety Standard for Rough Terrain Forklift Trucks (ANSI B56.6-1992)
