Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a)Table Mountain Casino expansion, first floor - Two Genie GS-3268 scissor
lifts were
being used to raise 22 ft and 25 ft re-shoring towers into the vertical
position.  This was
accomplished by resting or tieing one end of the re-shoring tower to the
guardrail of the
scissor lift and having the operator raise the platform.  Other employees
on the ground
would push the foot of the tower into place while it was being raised.
Use of this
personnel platform in this manner is not approved by the manufacturer and
recognized
as a tipping hazard.  On or about 10/18/00.
ABATEMENT NOTE:
Among other methods, one feasible method to reduce this hazard is to:
1.Follow national consensus standard ANSI/SIA A92.6 and manufacturer's
operation
manual, and
2.Use all self-propelled elevating work platforms (scissor lifts) only for
the purpose for
which they are designed.  Prohibit use of scissor lifts as material
lifting devices.
ABATEMENT DOCUMENTATION AND VERIFICATION IS REQUIRED.ION
