were causing or likely to cause death or serious physical harm to

were exposed to the possibility of being pinned under, crushed, or thrown
from a forklift during
an accident with the truck due to not wearing the seat belt that was
provided as required in
ASME B56.1-1993:
(a)Throughout the workplace, where employees operate a Toyota forklift
truck that was equipped with a seat belt, without utililzing the seat
belt, on or
about 6/15/00 and 6/21/00.
Among other methods, one feasible and acceptable method to correct this
hazard
is to train employees and insure that employees use the seat belts
provided when
operating a forklift truck as referenced in ASME B56.1-1993.
